1. What’s Included
• A £299 one-off annual membership fee, payable upon activation (non-refundable once
activated).
• GP consultations at a discounted rate of £9 per 15 minutes. Additional consultation
time may be subject to further charges.
• Prescriptions issued during consultations are included in the membership.
• Discounts of 10–15% on eligible Future Care Medical services during the active
membership period.
• Standard clinic charges apply for treatments, diagnostics, or medications not explicitly
listed under this plan.

2. What’s Not Included
The membership does not cover:
• Blood tests, imaging, or any services provided by external providers.
• Specialist referrals outside of Future Care Medical.
• Consultant dermatology services.
• Private medical reports, extended certificates, or letters not issued as part of routine care.

3. Key Terms
• Membership is valid for 12 months from the date of activation.
• The membership fee is non-refundable once activated.
• Membership benefits apply only to the registered individual and are non-transferable.
• Discounts cannot be used in conjunction with other offers, promotions, or packages.
• Membership is intended for reasonable personal use and may be reviewed if usage is
deemed excessive.

4. General
• Future Care Medical reserves the right to amend or withdraw membership benefits by
providing reasonable notice.
• All services remain subject to availability and the clinical judgment of our healthcare
professionals.• Membership does not create a contractual obligation for Future Care Medical to provide
services beyond those reasonably required or clinically appropriate.
